Occupational Therapy & Functional Assessments

Understanding function. Building independence. Providing evidence for the right supports.

Our Occupational Therapists focus on how disability affects a person's ability to participate in everyday life. We consider what the participant can do, what they find difficult, their current supports, environment, risks, routines and goals. Intervention may address personal care, dressing, meal preparation, household activities, shopping, time and money-management routines, planning and organisation, executive functioning, emotional and sensory regulation, community participation, independent living skills, safety awareness and environmental accessibility. We can also develop practical strategies such as visual schedules, step-by-step routines and environmental prompts to support participation and consistency.

Social Work Support & Therapy

Supporting wellbeing, resilience, relationships and meaningful participation.

Disability does not exist separately from the rest of a person's life. Relationships, housing, family circumstances, mental health, trauma, social connection, financial pressures, culture and access to services can all influence wellbeing and participation. Our Social Workers take a holistic, strengths-based approach and can assist with psychosocial assessment, emotional support, adjustment to disability, self-advocacy, relationships and family support, problem solving, community connection, crisis-related support, service navigation, informal support networks and multidisciplinary collaboration.

Positive Behaviour Support

Understand behaviour. Build skills. Improve quality of life.

Behaviours of concern can be influenced by disability, health, communication, trauma, environment and support needs. We work with participants, families and professionals to understand what behaviour is communicating and improve quality of life. Our Positive Behaviour Support includes assessment, behaviour support planning and proactive strategies. We help families and support teams implement effective strategies. We monitor outcomes and adjust supports as needs change. We promote positive alternatives to restrictive practices while prioritising safety and wellbeing. Our Complex Care approach coordinates support for participants with multiple or challenging needs. We bring services and professionals together to reduce fragmented care. We also support safe transitions between hospitals, justice settings, accommodation, SIL providers and community supports.

Support Coordination
SC
Support Coordination & Specialist Support Coordination

Making complex systems easier to navigate and supports easier to coordinate.

We help participants understand and implement their NDIS plans, identify priorities and connect with suitable providers. We coordinate services, monitor supports and address gaps or barriers. For participants with complex needs or higher risks, we provide more intensive specialist coordination. This may involve service breakdown, housing instability, hospitalisation, psychosocial disability, behaviours of concern or justice-system involvement. We bring providers and stakeholders together to improve communication and clarify responsibilities. We also support plan reassessments, changes in circumstances and complex transitions. Our focus is building stable, coordinated and sustainable supports while strengthening participants' confidence and independence.
