Our approach is personalised to each individual’s unique needs and circumstances. The process begins with thoroughly assessing the participant’s current situation and the upcoming transition. From there, a dedicated team collaborates with the participants, their families, and other stakeholders to create a tailored transition plan. This plan outlines the steps, resources, and support required.
